In Fit Model or Fit Proportional Hazards, add the factors you want to adjust to the "Construct Model Effects" list.
In the figure below, in addition to "treatment," "age" and "months" are included as adjustment factors (covariates) in the "model effect configuration" of the proportional hazards model.
