In short, you will want to make one small change to the code in the JMPAPPSOURCE for the DB with interface file to identify the name and location of the SQLite database on your system. This will let you avoid having to do it each time you run the file. I will not include the JMPADDIN file because of this reason. Your better off building your own Add-In once you have corrected the JMPAPPSOURCE file.

It has additional information about setting up the databases for both the Mac and Windows (including drivers for the Mac). Please check it out and let me know if you run into difficulties.
