Build custom UI into report

All, 
     I am trying to build an application using an UI that I have made previously which has a collection of panel boxes and list boxes. The easiest way it looks like to build the application is to have it in an open report. 

     Can someone provide an example of how to build a custom UI and use it in an application ? 

      For e.g. if I had the following display box, I wanted to build this using a script, how would I put it into a report, so I can drag and drop it into an application builder window ?

I don't know about drag and drop, but if you really want to avoid re-creating the UI in Application Builder, you could probably create a container box for that UI, and then in the module script, append all those elements to that container display box.
I attached an example where I did that.  I created an outline box named "ob1".  Then in the script for the module, I did "ob1 << Append()" and just pasted everything inside the New Window into Append().
